A SLAP tear or lesion occurs when there is damage to the top of the labrum, or where the biceps tendon connects to the labrum. The shoulder is one of the most complex joints in the body; it is held together by an intricate network of tendons, ligaments and soft tissue.

The labrum is a piece of cartilage that serves an important function in this network. See Soft Tissues of the Shoulder. The humerus head, or the top of the humerus bone in the arm, fits into the glenoid—but it is not a tight fit because the humerus head is usually larger than the glenoid socket.

The eccentric biceps activity during deceleration may serve to weaken the biceps-labrum complex, while the torsional peel back force may result in the posterosuperior detachment of the labral anchor.
